本人____________擔任 [XXX活動英文名稱]（[YYY活動中文名稱]）之活動參與者，透過此授權書同意[ZZZ主辦法人組織]與相關合作團隊，將本人提供之活動簡報或錄影檔案，或依據錄影內容製作之逐字稿，以下列勾選之授權或貢獻方式，代為本人標記後進行公眾授權模式之散布，或公開演播模式之揭露；並同意透過「上載錄影檔於 YouTube 線上影音平台」、「Facebook線上串流頻道」等線上公開網站平台之方式，以實作之。

本授權條款允許使用者重製、散布、傳輸以及修改著作（包括商業性利用），惟使用時必須按照著作人或授權人所指定的方式，表彰其姓名。

License Agreement
for Presentation Slide and Video Recording
[XXX-Full name of the speaker] (the “Licensor”) is invited to speak on “[YYY-Topic of the session given by the speaker]” at [ZZZ-Full title of the event] ([AAA-Abbreviation of the event]) in [BBB-Venue], [CCC-Country].
The Licensor agrees that his/her speak shall be recorded by the staff of
[AAA-Abbreviation of the event]. And the video file, transcript of the recording, or presentation slide checked below will be uploaded onto YouTube, Facebook or other applicable hosting platforms on his/her own name, under the chosen license or statement appointed by the licensor.
